Document Supply

To be able to request copies of documents (for example parts of books or journals) electronically you must first register as a current Oceania University of Medicine student/staff member.

Please print, read, sign and date the Document Supply Registration Form and submit it by fax, post or in person to:

This is required by the Australian Copyright Act 1968. By signing this form you are authorising the Library to accept your Staff/Student Username/Password as your "electronic signature" for copyright purposes for all future requests.

Once we have received your registration you may request the library staff to supply you with documents by completing a Document Request Form online for each item you request.

Oceania University of Medicine complies with the provisions of the Australian Privacy Act 1988 in regard to handling any personal details you supply.

Note: the Registration form needs to be completed only once but the declaration you make in this form to abide by Section 49(1) of the Australian Copyright Act 1968, is taken to be repeated each time you request a copy of an article or other work using the Oceania University of Medicine Document Request Form.
